Fees

Individuals:

$175 per 50-minute session.

Couples/Families:

$225 per 50-minute session.

Payment

Cash, checks and credit cards are accepted. Payment is due at the start of the session. I require a valid credit card number on file.

Cancellations

If you need to cancel, please notify me by phone-call 24 hours before your scheduled appointment. Because your time is specifically reserved for you, cancellations less than 24 hours in advance compromise my ability to support other clients and will result in a charge on your credit card of your full fee for the missed session.

Insurance

I am currently on the Anthem Blue Cross preferred provider panel.

Anthem Blue Cross Members:
Please confirm with Anthem Blue Cross whether or not you need an authorization to see me for counseling. Please also bring in your Insurance ID Card to our first appointment.

Please be advised that until you meet your deductible you will be charged the current fee that Blue Cross covers for therapy services. Once your deductible has been met, the insurance company will send an Explanation of Benefits, at which point you will be responsible only for the co-pay until the end of the calendar year.

You are responsible for my full fee of $130 for a no show or same day cancelation.

For Other Insurance Plans:
I do not deal directly with any other insurance companies. However, many PPOs offer reimbursement for out of network mental health providers. I will provide you with monthly statements so that you are able to seek reimbursement from your insurance company. It is your responsibility to pay our agreed upon fee for services at the time they are rendered, regardless of your insurance arrangements. Services may be covered in full or in part by your health insurance or employee benefit plan.

Please check your coverage carefully by asking your insurance provider the following questions:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?
• What exact information is necessary to submit for reimbursement (i.e diagnosis code, provider tax ID, etc.)?
